The secure enrollment platform where you complete intake and manage your membership is governed by the terms and notices presented to you there.

This marketing website does not ask for health information. Please do not send health details through our contact form.

Health information you provide for an evaluation is collected through our intake system and passed to the treating professional entity. Clinical records created in the course of your care are held by that entity as the covered entity under HIPAA.

We ask for your separate, specific consent before collecting consumer health data at intake, and again before sharing it with anyone other than the treating professional entity and the vendors needed to deliver your care.

We never sell consumer health data, and we do not use it for advertising.
